THEREFORE, you are ordered to:

  • produce copies of the records that are in your possession or control once you have received this order;
  • the records must be produced to [name of applicant] at [office address] within 30 days of service in the following form:
    • readable and usable form with a certified affidavit attached attesting to the records being a true and complete copies of the records responsive to the order;

This order is subject the conditions as follows:

  • you have the right to apply to revoke or vary the order;
  • contravention of the order without lawful excuse may result in penalties including a fine, imprisonment or both.
